In my undergraduate studies, I have taken one full time year (defined as courses per semester), one year of 5/4, one year of 4/4, and one year of 2/4 as I was making a program switch. Next year I will be reverting to 5/5. My GPA is a perfect 4.0. I just worry that they’re not going to be able to look past my course load. I have a variety of extracurricular activities, volunteer positions, and leadership positions.

NO, you should have a good chance.
That 4.0 average is impressive if it includes the science courses that are often the back breakers for pre-med students - like organic chemistry.
I did not overload my schedule as a pre-med student in the 1970's.
I had no trouble getting into the med school of my choice with a 3.85 GPA.
